How many seniors experience incontinence? Incontinence is the loss of bladder/and
or bowel control. Other causes of incontinence include infections, damage to the pelvic
muscles, medication side effects (diuretics), diabetes or obesity, birth defects, injury,
stress or pregnancy, delirium, stool impaction, stones in the urinary tract, and prostate
problems with men or disorders of the pelvic floor in women. 61% of
those with urinary incontinence also have faecal incontinence.

People with urge incontinence lose urine as soon as they feel a strong need to go to
the bathroom. If you have urge incontinence you may leak urine: When you can't get to the
bathroom quickly enough When you drink even a small amount of liquid, or when you hear or
touch running water You may also Go to the bathroom very often; for example, every two
hours during the day and night. If you have stress incontinence, you may leak urine: When
you sneeze, cough, or laugh When you get up from a chair or get out of bed When you walk
or do other exercise You may also Go to the bathroom often during the day to avoid
accidents If you have overflow incontinence, you may: Often lose small amounts of urine
during the day and night Get up often during the night to go to the bathroom Often feel as
if you have to empty your bladder but can't Pass only a small amount of urine but feel as
if your bladder is still partly full Spend a long time at the toilet, but produce only a
weak, dribbling stream of urine Some people will overflow incontinence do not have the
feeling of fullness, but they lose urine day and night. Surgery can be used to: Return the
bladder neck to its proper position in women with stress incontinence Remove tissue that
is causing a blockage Replace or support severely weakened pelvic muscles Enlarge a small
bladder to hold more urine There are many different surgical procedures that may be used
to treat incontinence. Three types of treatment are recommended for urinary incontinence:

Angie, and Susan - and we also have a great mix of community contractors and volunteers

Researchers estimate the nation spends at least $16 billion a year to care for
people with urinary incontinence, up from $10 billion in 1990. (symbol: MCAR) has
developed a cost effective, non-surgical, non-pharmacological and non-invasive system for

lack basic knowledge of the condition & treatment methodologies available today that
can offer that glimmer of hope to the sufferer. The mission of the Society for Continence
(S'pore) is to rectify these misconceptions & educate & inform each & everyone
on what is incontinence, how we diagnose this condition, what help is available &
where to find help and from whom. The providers of the clinical care of urinary
incontinence in Singapore come from Urologists, Paediatricians, Geriatricians, Community
Health Physicians & Primary Care Doctors including General Practitioners,
Gynaecologists, Rehabilitation Specialists, Behaviourists, Pharmacists, Physiotherapists
and most importantly Continence Care Nurses. Together this team has initiated the first
ever professional & patient support group for this condition with encouragement &

A new UI treatment guide for caregivers --such as certified nursing assistants in
long-term care facilities and home health aides--is now available. This, and a companion
piece (the Alert) for directors of nursing to use in training caregivers, are based on a
clinical practice guideline sponsored by the Agency for Health Care Policy and Research
(AHCPR), released earlier this year. Through a partnership with the American Health Care
Association (AHCA) and the American Medical Directors Association (AMDA), the new
caregiver guide and Alert were developed and will be disseminated to medical directors and
other professionals in long-term care facilities. The caregiver guide stresses the need to
maintain a regular toileting schedule and provide aids such as walkers and toilet supports
to help people get to and use the toilet. The caregiver guide, Helping People With
Incontinence, and the companion piece for directors of nursing, Alert: Establishing,
Implementing, and Continuing an Effective Continence Program in a Long-Term Care Facility,

The clinics offer a new cost-effective program of treatment developed by MedCare
Technologies. "These clinics address an absolutely enormous need," stated
Valerie Boeldt-Umbright, Director of Clinical Services for MedCare. "The sad fact is
that incontinence is surprisingly widespread yet the vast majority of sufferers have yet
to seek treatment." A study completed at Emory University in Atlanta, GA, concluded
that the prevalence of urinary incontinence (UI) was 43.6% among women and 20.9% among
men. A study in the United States of 1,150 women aged 60 years and older indicated that
38% experienced at least one incontinent episode during the previous 12 months. Stress
incontinence is seen predominantly in women under 60 and is often caused by a decrease in
pelvic muscle strength due to childbirth, surgery or reduced hormones associated with
menopause. They plan to open clinics in U.S. cities starting in Overland Park, KS, Norman,
OK, and Chicago in October and November.

Some of the most important "breakthroughs" in the field of biofeedback have
come not from one definitive study but from the accumulation of numerous studies over many
years. Rather than focusing only on the latest published articles, we felt it important to
place the field of biofeedback in its historical context. Back in the 1960s when
experimental psychologist Neal Miller first demonstrated that the autonomic nervous system
could be trained to alter some bodily processes, it was thought biofeedback would change
the world. While biofeedback has been used successfully to treat some psychological and
mental disorders, it seems to work best with patients in which physiological processes are
relevant (Futterman & Shapiro, 1986). The most common forms of biofeedback today make
use of instruments to "feed back" information about such bodily processes as
muscle tension (EMG feedback), skin temperature (thermal feedback), brain waves (EEG
feedback) and respiration. ) The researchers' treatment protocol includes 6-8 weeks of
thermal biofeedback and autogenic training, followed by 30 sessions of evoked images of
personal change and alpha-theta EEG biofeedback.

may benefit from practicing a series of pelvic muscle exercises, with or without weighted
vaginal cones, designed to strengthen the muscles of the pelvic floor. Depending on the
problem, there are medications which relax the smooth muscles of the bladder wall or
strengthen the sphincter muscles responsible for holding urine in the bladder in patients
with stress incontinence. After controlling for factors such as age, overall health, and
education, Herzog and her colleagues found that there was no link between urinary
incontinence and death--specifically, the condition had no effect on 6-year mortality
among these community-dwelling individuals.

In men, stress incontinence is seen occasionally following prostatectomy or trauma to
the membranous urethra or bladder neck. Total incompetence of the urinary sphincter is
characterized by a constant involuntary dripping of urine from the urethra day and night
without bladder distention, as determined by physical examination of the abdomen and
measurement of residual urine. In children, congenital failure of the urethra to close
properly (eg, epispadias) results in this form of incontinence. Urinary fistulas in women
cause involuntary urine loss from the urethra or, most commonly, from the vagina. Such
fistulas usually are secondary to operative trauma, neoplasms, radiation therapy,
automobile accidents, traumatic childbirth, or gunshot wounds, in which a tract becomes
established and conducts urine from the ureter, bladder, or urethra into the vagina. A
child may have incontinence secondary to both an uninhibited neurogenic bladder
dysfunction and psychogenic factors; an adult male may have both overflow incontinence due
to prostatic obstruction and neurogenic bladder dysfunction secondary to diabetic
neuropathy or cerebrovascular insufficiency.

There are several externally worn appliances that can be used to protect the skin from
the constant leakage of liquid stool or urine. These appliances are usually referred to as
external incontinence devices. Liquid stool is very caustic to the skin and may lead to
skin excoriation and breakdown. Often a protective skin barrier is applied to the skin
prior to attaching the device. You can also use a skin barrier paste to fill in the skin
fold of the buttox to prevent leakage of liquid stool through this area. DEVICES FOR
WOMEN: External collection devices are available for women, although these devices are
less successful in containing urine.

The Medtronic Interstim Sacral Nerve Stimulation (SNS) System was evaluated in a
multicenter trial at study centers in the United States, Canada, and Europe for the
indication of urinary urge incontinence. 155 urge incontinent patients were enrolled in
the study (30 males) Mean age was 47 years (range 20 to 79 years). These 155 patients
underwent at least 1, and, in some cases, up to 5 Peripheral Nerve Evaluation (PNE)
procedures. 58 patients (6 males) have data at 6 months follow-up and 38 patients (4
males) have data at 12 months follow-up. Patients were randomized into either an immediate
implantation of the Medtronic SNS System (treatment arm) or 6-month delay from implant
(control arm). Voiding diaries were completed at baseline and at 6 months for control
group patients and at baseline, 1, 3, 6, and 12 months post implant (and at 6-month
intervals thereafter) for the treatment group patients.

Urinary Incontinence (UI) is the leakage or loss of urine when you don't want to; a
very common problem in the United States estimated to afflict 13 million Americans. In
addition to the patient, UI is also a burden for family caregivers and the community, and
the major reason aging parents are put into nursing homes. A number of other factors have
been associated with UI, such as urinary tract infections, menopause, obesity, multiple
childbirths, episotomy, lack of postpartum exercise, impaired mental functioning,
constipation, fecal impaction, and skin problems. Approximately 30 to 50 percent of people
who suffer from UI do not report their problem to a doctor or nurse and when reported,
many physicians and nurses fail to help the person. Instead, many people with UI turn
prematurely to the use of absorbent products sold in drug stores without having their UI
appropriately diagnosed and treated. The odor of urine that permeates many nursing homes
can repel residents, staff, and visitors; although nurses, families, and even residents
seem to tolerate UI as a normal consequence of aging.

In straightforward cases surgical intervention is 90-95% effective with 85% of cured
patients still dry five years later. Non-surgical therapies such as medications, pelvic
floor exercises or pelvic floor stimulation have varying degrees of success depending on
the patient's pretreatment condition. Fifty per cent of individuals with straightforward
genuine stress incontinence or detrusor instability can expect improvement or cure using a

Kegel, or pelvic floor muscle exercises are done to strengthen the muscles which
support the urethra, bladder, uterus and rectum. Often the pelvic floor muscles are weak
which contributes to problems with losing urine. Doing the exercises correctly and
regularly can strengthen the muscles. Stronger muscles lead to little or no urine loss for
many women. We recommend doing the exercises for five minutes twice a day. Once the
exercises become easy, you can further strengthen the muscles by squeezing to a count of
eight and relaxing to a count of eight.

The lower part of the urinary tract consists of a hollow muscular storage organ called
the bladder, that is found in the pelvis behind the pelvic bone (pubic symphysis) and a
drainage tube, called the urethra, that exits to the outside of the body. The urinary
bladder, ureters, and urethra are all muscular structures lined with a membrane coated
with mucus that is impermeable to the normal soluble substances of the urine. The urethra
is a small tube which leads from the floor or neck of the urinary bladder to the outside
of the body. The mechanism responsible for supporting the bladder neck and urethra
involves the interconnections of three structures: (1) the arcus tendinous fasciae pelvis,
(2) the levator ani muscles, and (3) the endopelvic fascia around the urethra and vagina.
Urinary bladder function not only involves smooth and striated muscle systems but also a
complex combination of nervous system components located in the brain and along the spinal
cord as well as in the bladder and urethra. Urinary continence is maintained as long as
the pressure within the urethra (intra-urethral pressure) remains higher than the pressure
within the cavity of the bladder (intravesical pressure).

In 1992, the Agency for Health Care Policy and Research (AHCPR) released its first
guideline on urinary incontinence. Since then the guideline has become the standard of
care for incontinence in many settings across the country. This update of the guideline
reflects new findings in the rapidly changing field of treatment for urinary incontinence.
To develop the update, AHCPR convened a multidisciplinary private-sector panel of
physicians, nurses, allied health professionals, and health care consumers to study the
effectiveness of diagnostic and treatment procedures for urinary incontinence, their

In the United States, at least 13 million people have problems holding urine until they
can get to a toilet. This loss of urinary control is called "urinary
incontinence" or just "incontinence. " Although it affects many young
people, it usually disappears naturally over time, which suggests that incontinence, for

incontinence depends on making the most accurate diagnosis of the type of incontinence

First of all, if the "incontinence" is relatively new, it may not be
(chronic) incontinence at all, but simply a temporary by-product of another condition,
such as an infection, an injury, constipation, or even a side-effect of a prescription

of pelvic muscles. The entire field of pelvic muscle rehabilitation in the treatment of
urinary incontinence has its origins in the work of a single physician, Dr. Since March,
1992 when the Agency for Health Care Policy and Research officially recommended that
behavioral methods including biofeedback be utilized before drugs and surgery for urinary
incontinence, most clinicians have used these Guidelines to document and justify opening
"incontinence clinics". Many early attempts to use biofeedback in the treatment
of incontinence were only marginally successful because clinicians were not able to
generate enough motivation in their patients to reach significant levels of exercise; so

Nursing Research, the National Institute of Neurological and Communicative Disorders and
Stroke, and the Veterans Administration, convened a Consensus Development Conference on
Urinary Incontinence in Adults on October 3-5, 1988. After a day and a half of
presentations by experts in the relevant fields involved with urinary incontinence, a
consensus panel consisting of representatives from geriatrics, urology, gynecology,
psychology, nursing, epidemiology, basic sciences, and the public considered the evidence
and developed answers to the following central questions:What is the prevalence and
clinical, psychological, and social impact of urinary incontinence among persons living at
home and in institutions? The most commonly encountered clinical forms of urinary
incontinence in adults are stress incontinence, urge incontinence, overflow incontinence,
and a mixed form. Examples of patients rarely requiring further diagnostic testing after
the core examination include the young woman with classic stress incontinence or the
80-year-old woman with a recent stroke and the new onset of urge incontinence. Several
studies have shown no improvement in stress incontinence, but women with postmenopausal
urge incontinence, urgency, and frequency have shown improvement. When used in patients
with stress and/or urge incontinence, biofeedback has been shown to result in complete
control of incontinence in approximately 20-25 percent of patients and to provide
important improvement in another 30 percent.

Bladder Awareness Campaign Encourages Women to Seek Treatment Women need to know they
don't have to live with urine leakage or loss of bladder control any longer because many
treatments--from pelvic floor exercises to surgery--are available. That's the message the
National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) of the National
Institutes of Health (NIH) and a partnership of professional and patient advocacy groups
concerned with urinary incontinence are highlighting in Let's Talk About Bladder Control
for Women," a new campaign to make women aware that this costly and embarrassing
condition is treatable. About 11 million of the 13 million Americans affected by urinary
incontinence are women, according to the Federal government's Agency for Health Care
Policy and Research (AHCPR). Through a recent survey on women and incontinence, we learned
that a quarter of the women surveyed feel embarrassed about urinary incontinence and are
uncertain about dealing with the condition in social situations. The Let's Talk About
Bladder Control for Women awareness campaign offers easy-to-read booklets explaining the
symptoms, types and causes of poor bladder control as well as treatment options. The core
partners in Let's Talk About Bladder Control for Women are the American Uro-Gynecologic
Association, American Urological Association, American Foundation for Urologic Disease,
National Association For Continence, Society of Urologic Nurses and Associates and the
Simon Foundation for Continence.

The latest minimally invasive procedure available for stress incontinence is
Videoscopic "Balloon" Bladder Neck Suspension. "Minimally Invasive"
Surgery- Recently, a new minimally invasive technique for stress incontinence called
Videoscopic "Balloon" Bladder Neck Suspension is now being preformed to modify
the bladder neck position. This procedure can cure stress incontinence with success rates
of 92-94% having been reported. Most women undergoing this procedure go some the same day
of surgery. It may include:
Diet- avoid drinking coffee, colas, and alcohol. This
procedure is major surgery and is usually reserved for women with Type III incontinence
and/or "mixed" incontinence where the patient's sphincter muscle no longer

for the past two decades to treat breast cancer. Immunex Corporation recently announced
EPIC -- Empowered Patients In Control, an education program for late-stage prostate cancer
patients. Patients and supervisors had widely different views of how cancer's side effects
affect job performance; 85 percent of supervisors cited patient fatigue, while only 58
percent of patients said they suffered from it.

Urinary incontinence (in-CON-ti-nents) is the loss of bladder control or the leakage of
urine. Urinary incontinence is a major health problem because it can lead to disability
and dependency. The most common types of urinary incontinence are: Stress incontinence
happens when urine leaks during exercise, coughing, sneezing, laughing, lifting heavy
objects, or other body movements that put pressure on the bladder. Although healthy people
can have urge incontinence, it is often found in people who have diabetes, stroke,
dementia, Parkinson's disease, or multiple sclerosis. Functional incontinence happens in
many older people who have relatively normal urine control but who have a hard time
getting to the toilet in time because of arthritis or other crippling disorders.
Implanting devices that replace or aid the muscles controlling urine flow has been tried
in people with incontinence.
