URETHRAL SYNDROME Urethral syndrome refers to a primarily female syndrome in which women suffer from irritative bladder symptoms (urinary frequency, urgency, burning, and more) in the absence of any objective urologic findings. Because the symptoms of urethral syndrome are similar to symptoms common for many irritative bladder problems, the diagnosis of urethral syndrome is often delayed. Symptoms, Causes, and Diagnosis Once again, the irritative symptoms accompanying urethral syndrome include: urinary frequency, urgency and burning in addition to possible low buck pain, suprapubic pain, and hesitant or slowed urinary stream. Because these symptoms are so common with other urinary problems, your physician must eliminate other disease possibilities and diagnose urethral syndrome by exclusion. Symptoms and Signs The classic signs and symptoms of NSU are burning upon urination and a light discharge. Causes and Treatment of NSU The most common cause of NSU is a microorganism called Chlamydia. | |||

Introduction This brief overview of Idiopathic Human Intersitial Cystitis is taken from the publication Infect. This information should be of use to veterinarians as a similar--possibly identical--syndrome (Feline Idiopathic Cystitis) has been documented in cats--some of whom have been erroneously diagnosed with a behavioral inappropriate urination problem. Interstitial Cystitis (IC) primarily affects women (approximately 90% of cases) though a related condition, Prostatodynia, occurs in men. Etiology Theories regarding the cause of human IC include autoimmune, infection, toxin, ischemia and defect in the bladder glycosaminoglycan protective layer synthesis. An antinuclear antibody has been found in the sera of patients with IC that matches one found in patients with suspected immune-mediated arthralgia/myalgia-- raising the issue of a possible common autoantigen. Infection There is no consistent correlation of urinary tract infection with IC, based upon cultures of urine and of bladder itself. | |||

In treating interstitial lung disease from most causes, your physician may prescribe one or more drugs which are immunosuppressive agents, that is they are drugs which help decrease the body's own inflammatory response. The four most commonly prescribed drugs are prednisone, cyclophosphamide, methotrexate and chlorambucil. Prednisone is administered in doses ranging from 20 mg to 60 mg per day initially, depending on which specific disease is being treated. After about 6 weeks, the higher dose prednisone therapy (60 mg/day) is reduced to a lower, less toxic dose. Cyclophosphamide or chlorambucil therapy may be employed in most interstitial lung diseases, either as an additive drug to the prednisone (ie:IPF), or as an alternative to prednisone (ie:sarcoidosis) when the physician determines that it is either not working as effectively as it might, or because of serious side effects of the prednisone. Methotrexate is often used as an additional drug with prednisone when lower doses of prednisone are desired. | |||

URINARY TRACT INFECTIONS IN ADULTS Infections of the urinary tract are common -- only respiratory infections occur more often. Microorganisms called Chlamydia and Mycoplasma may also cause UTI's in both men and women, but these infections tend to remain limited to the urethra and reproductive system. Usually, the latest infection stems from a strain or type of bacteria that is different from the infection before it, indicating a separate infection. A doctor suspects one of these infections when a person has symptoms of a UTI and pus in the urine, but a standard culture fails to grow any bacteria. ) If you're a woman, you've probably suffered (or suffer) from cystitis or have female friends who suffer. If you're a man, you've possibly suffered yourself, but more likely have had to deal with a woman in pain from cystitis or interstitial cystitis. As anyone afflicted with these difficulties or in partnership with someone who has them can tell you, cystitis in either of its forms can put a real crimp in your love life, causing painful intercourse for the woman or the frustration that comes of NO SEX or LIMITED SEX for both parties during a flare-up. Interstitial Cystitis (IC) , on the other hand is a chronic inflammation of the bladder wall. It's cause has been difficult for researchers to pin down, and there are many theories about where it comes from and what to do about the condition. Someone with an interstitial cystitis flare-up may or may not exhibit symptoms of common cystitis and may turn up free of bacteria on a urinalysis. Recent research indicates that IC may be related to allergies (especially food) of various types. Both kinds hurt like hell. IC hurts like hell recurrently, and, sometimes hurts like hell without cessation. Signs and symptoms of both types of cystitis can include: burning pain on urination lower abdominal pain frequent need to urinate (up to 60 times a day! I have suffered, as millions of other men and women, with recurring bouts of painful and debilitating cystitis and near chronic bladder/urethral pain. Add to that the pain of repeatedly disappointing an amorous partner and you begin to see that cystitis is not much fun. Partners begin to feel resentful, and occasionally harbor the suspicion that this IC stuff could be just another version of "not now, honey, I have a headache." IC has been known to destroy relationships. this is understandable, they have a sort of built-in interest in the how the male organs work. You would think that gynecologists, then, would be trained in female urinary functions. Well, no. I'm told they generally have no specialty training in that area. Though they know about and can usually treat common cystitis, few general practitioners are familiar with the signs, symptoms or diagnosis of IC. Many women see five or more doctors before getting a diagnosis, often giving up in frustration and suffering, well, if not actually in silence, certainly without much medical help. Additionally, the tests for diagnosing and techniques for treating IC are often invasive (scary and painful), can require general anesthesia and are expensive. HMO doctors, with their eye on the bottom line rarely want to refer you to the proper specialists to deal with the problem. After all, it is you, not them, who are in pain. Allow me a soap box for moment folks. The worst thing to happen to medicine in our time is the advent of managed care. Docs used to price-gouge us, yes, but at least they did their jobs. I have yet to visit one HMO physician whom I felt had any kind of interest in me or my health. The very best I have had was a female DO who would prescribe a requested antibiotic which I knew often helped. Well, I have located two organizations that provide assistance. The Interstitial Cystitis Association (ICA) is the most well-known organization paying attention to IC. They can provide you with a list of knowledgeable health care providers and connect with you with support groups in or near your geographical area. They also have a variety of information available to both sufferers of IC and their physicians. The other organization, The Interstitial Cystitis Network (ICN), provides numerous on-line support forums and other resources for IC sufferers. You can also check out other IC Web Resources. | |||

This condition is characterized by frequent bouts of symptoms of urinary infection (burning, frequency and urgency of urination) without a bacterial infection actually being present. However, as we get older, we learn to stop that involuntary urge to defecate or urinate (control the reflex) by clenching our external, striated (voluntary) muscles (the levator ani) in order to not embarrass ourselves in public (besides it is so messy and uncomfortable--and doesn't even smell so good). The elimination process takes place, the involuntary sphincters close (until the organs are once again full) and the voluntary sphincters are supposed to STAY relaxed until they are needed again. However, if a person is experiencing a continuous fight-or-flight stress effect in the hypothalamus (so that the body is ALWAYS "ready") ALL of the muscles in the body stay more tense than normal. However, more importantly, the increased pressure in the bladder forces urine into the bladder wall which causes "interstitial" (inside the wall) inflammation. In studies of pentosan polysulfate (Elmiron, Baker Norton) with those fitting NIHs definition of IC, 100 mg three times a day gave 38% of patients more than 50% improvement in bladder pain, vs. Though some patients respond quickly and dramatically, it may take three to six months for the drug to take effect. The drugmaker says that in unblinded studies about 50% of those who didnt respond to catheter-delivered DMSOthe only other medical therapy for ICdid respond to pentosan polysulfate. The drug, says the maker, will cost patients about $5 a day. | |||

A different culture (broth) may lead to the finding of elusive bacteria, and thus treatment may be undertaken to rid the bladder of the offending pathogen(s). A recent national study conducted by the Cystitis Research Center and the ICIC has shown IC to be common to all ages of women (10% of IC sufferers are men) with the average age at onset being 36-1/2 years. Gordon and associates[4] reported on a series of patients (n=16) who complained of joint and muscle pain and severe fatigue. Miller and coworkers[5] found petechial hemorrhages (glomerulations) of the bladder epithelium after hydrodistension in 12 of 20 patients with prostatodynia or nonbacterial prostatitis. Domingue and associates[10] also have contributed to the concept that IC is an infection not detectable by routine cultures. For refractory patients, intravesical sodium oxychlorosene (Chlorpactin) can be tried. These results were further supported by Hughes and associates,[25] who reported that after cystoplasty, 20 of 22 patients with a bladder capacity of less than 250mL (small capacity) under anesthesia were either cured (15 patients) or showed improvement of symptoms (5 patients). | |||

Interstitial cystitis (IC) is a condition characterized by inflammation and irritation of the bladder. NEW YORK (Reuters) -- A common, chronic bladder problem in women -- interstitial cystitis (IC) -- is not caused by bacteria, a new study suggests. ``Until now, there has been some controversy as to the role played by bacterial infection in IC,'' says Dr. Since we used such a sensitive assay and a control group (of people without the disorder), we believe more than ever that IC is not caused by bacteria.'' The assay Keay refers to is a DNA test capable of finding one-half of one bacterium on a sample of bladder tissue obtained through cystoscopy. The bladder is then distended with air, allowing the doctor to inspect its wall for signs of IC, such as inflammation and ulcers, and to snip off a sample of tissue. No difference in bacterial types was found between the IC patients and those without the disorder, ``providing additional evidence that IC is not associated with infection by a particular type of bacterium,'' Keay and her colleagues state. Keay says she hopes her ongoing studies into IC will yield new clues as to the nature of the disorder. | |||
